Selecting and Changing Your Primary Care Physician
To choose your Primary Care Physician (PCP), PLEASE NOTE the following steps:
Choose a specific Physician Code (the number listed next to the physician's name) from the list of physicians included in this brochure.
Place the Physician Code in the appropriate space on the enrollment form.
You may choose to have a different physician for each member of your family, but you must write the Physician Code by each family member's name on the enrollment form.
Female members, age 14 and over, must also select an obstetrical and gynecological (OB/GYN) provider from one of HPN's OB/GYN physicians.
Once you choose your PCP, you must seek all care from that physician. He/She will supervise all of your medical care.
If you wish to change your PCP, you must contact the Member Services Department at (702) 242-7300 or (800) 777-1840 (TDD/TTY at (702) 242-9214 or (800) 349-3538), Monday through Friday, 8 a.m. to 5 p.m. If you submit your change request by the 20th of the month, your change will be effective for treatment received by the new PCP the first day of the following month. If you submit your change after the 20th, you will not be effective with your new PCP until one full month after the following month.
It's important to know that when you enroll in an HPN plan, services are provided through HPN's delivery system, but the continued participation of any one doctor, hospital or other provider cannot be guaranteed. If you have questions regarding claims, please contact the phone number listed on the back of your insurance identification card.
If specialty care is needed, your PCP must refer you to a designated specialist. Failure to follow the requirements of this referral process will result in higher out-of-pocket costs to you. Visits to a specialist without the required referral will be denied by HPN, and you will be responsible for the costs.

Senior Dimensions Members:
Coverage under the Senior Dimensions plan includes access to emergency services, urgently needed services, and out of area dialysis services. These services will be covered and can be obtained without a PCP referral or authorization. In the event of an Emergency Medical Condition, go to the closest emergency room, or where available, call 911 for assistance. Health Plan will cover Emergency Services whether you are in or out of the service area. You should have someone telephone Health Plan at the number listed on your membership card within 48 hours; or as soon as reasonably possible when an emergency occurs so that we can be involved in the management of your health care and transfer can be arranged when your medical condition is stable (depending on the distance involved).
Providers should submit bills to Health Plan directly for payment. However, if you paid for any Emergency Services, Urgently Needed Services, or out of area dialysis services obtained from Non-contracted Medical Providers, you should submit your itemized bills to Health Plan for payment. You should be prepared to assist Health Plan in obtaining all information necessary to properly process your request. Bills should be submitted to the following address:
